Support Coordinator
Vish is a community and health care professional with a Clinical background and experience working
in the disability industry for the last 4 years in various capacity building and NDIS consultant roles in the community, mental health and hospital settings.
Vish is equipped with expertise in the NDIS space, certifications and in mental health and application of the NDIS Support Framework. His learned experiences include navigating support for persons identifying with permanent disability to achieve their NDIS plan goals.
His approach has always been active listening, polite with a person-centred focus to plan and
implement supports.
Vish has an additional Role as an Autism advisor with ongoing development of the scope of services
for the community of persons with Autism.

Understanding the participant and NDIS plan goals, inputs from delegate/area
coordinator/referee/family/carers
Initial assessment and screening
Engagement with the Participant and connecting with their chosen supports as relevant
Development of a Support Plan – NDIS Support Framework
Schedule of supports – as chosen
Service Delivery and Reporting
Sessions as scheduled and progress notes and reports to NDIA
